How To Prepare For Your Chiropractic Appointment
There is not a whole lot you need to do in terms of preparing for your chiropractic appointment, but here are a few considerations for you to ponder prior to your initial visit with us:
- You can download and fill out our Adult Intake Form located below. You can either fax them or bring them with you to your initial appointment. This will save you about 20-30 minutes. We also provide separate forms for infants and children if you need to bring in those most precious to you due to issues like Colic, bed wetting, and poor digestion.
- Bring your insurance information.
- Do try to avoid strenuous activity before your visit (heavy lifting, awkward bending and twisting, intense physical activity like contact sports, etc.).
- Be prepared to discuss your pain, injuries, symptoms, health conditions, etc. It may help you to take a few notes to help you remember any questions or concerns you may have for the chiropractor, and notable information that can better assist us in optimizing your chiropractic treatments (e.g. lifestyle practices, medications, supplements, allergies, current injuries, previous medical and alternative medical treatments, etc.). If you have had recent radiological imaging (X-ray, MRI, or other), feel free to bring those scans to the office if you have them.
How To Prepare For Your First Massage
There is not a whole lot you need to do in terms of preparing for your first massage, but here are a few considerations for you to ponder prior to your initial visit with us:
- Please download and fill out our Massage Intake Form and Cancellation Policy located below. You can either fax them or bring them with you to your initial appointment. Please arrive 5-10 minutes early to your massage if you do not want to fill out your paperwork ahead of time.
- If you have any major medical issues or concerns that our therapists should know about, please call the office at (970) 587-7029 and let us know ahead of time.
- Bring a credit card with you to the office, we do require one on file.
- Do try to avoid strenuous activity before your visit (heavy lifting, awkward bending and twisting, intense physical activity like contact sports, etc.).
- Be prepared to discuss your pain, injuries, symptoms, health conditions, etc. It may help you to take a few notes to help you remember any questions or concerns you may have for your massage therapist, and notable information that can better assist us in optimizing your visit.
- Drink plenty of water to keep your tissues hydrated prior to a massage. Many patients prefer to bring a water bottle along with them to hydrate after a treatment.

– Lisa is trained in Deep Tissue, Neuromuscular Therapy, Prenatal Massage, and many other modalities. She is passionate about massage as a whole, and after 7 years in the industry, she is able and willing to tailor her sessions for each clients’ specific needs and wants.
– Claire would describe her style as relaxing and therapeutic. She performs Swedish, Deep Tissue, Trigger Point, Prenatal, Sports, and various other modalities. Her favorite area of the body to work is the neck. She likes to use her tool belt of modalities, created from 6 years in the industry, to tailor each session to the individual client to achieve her goal – that each person leaves relaxed and with less pain.
– Ashly focuses on a blend of relaxation and structural/alignment-based techniques, including Deep Tissue, Swedish, Trigger Point, Sports, and many others. Her technique, which has been developed over 11 years, uses deeper strokes to work out the knots and tight fascial tissue that can cause pain and loss of space/movement, and lighter strokes, rocking, and movement to relax the muscles and nervous system. She uses compression and stretching to build muscle memory. She specialized in shoulders, hips, and plantar fasciitis.
